Cardiac Rehabilitation

Kishwaukee Community Hospital offers a comprehensive exercise, education and support program for patients with known heart disease. The cardiac rehab program is medically supervised and individualized to assist the patient in reaching and maintaining an optimal state of physical, psychosocial and vocational well-being. The cardiac rehab team is a multidisciplinary team including registered nurses, an exercise physiologist and an EMT. Patients also have access to a respiratory therapist, a pharmacist and a dietitian as needed. The team assists patients in goal setting, lifestyle modification and education. A physician’s order is necessary for each phase of the cardiac rehab program.

Kish Hospital offers three phases of cardiac rehabilitation:

Inpatient (Phase I)

While the patient is hospitalized, a structured exercise and education program is designed for the individual. When the physician orders Phase I Cardiac Rehab, the program is started immediately at the hospital, if the patient is physically able. Many patients do not have Phase I since inpatient times are often very short. A patient is started with Phase II if he or she has an appropriate diagnosis.

Immediate Outpatient (Phase II)

The primary goal of the immediate outpatient program is to assist patients in achieving and understanding their safe maximum exercise level within the limits of their disease as well as to educate and assist with risk factor modification to prevent future heart disease.

The patient receives a comprehensive initial evaluation and risk stratification. Determined by patient need and risk, the patient comes in to the cardiac rehab facility to exercise 3 times per week for approximately 1 hour with cardiac monitoring throughout the session for a maximum of 12 weeks. Weekly 60-minute educational classes are included in the program. Covered diagnoses for Phase II include: myocardial infarction, coronary artery bypass graft (CABG), stents, heart transplant, valve surgery, percutaneous transluminal coronary angioplasty (PTCA), and angina.

Maintenance (Phase III/IV)

The primary goal of the maintenance program is to further develop and maintain cardiovascular endurance and decrease risk of future heart disease with a supervised exercise and educational program. Any patient with any kind of cardiac diagnosis is eligible for Phase III/IV.

Education

The cardiac rehab program offers a core rotation of class topics with additional topics available based on patient needs.

The class topics are:

  • Anatomy and Physiology

  • Risk Factors

  • Diabetes

  • Intimacy

  • Diet

  • Exercise

  • Medications

  • Stress and Stress Management
